  "text": "(1) The Appendices to Annex 2 are amended as follows.\n(2) In Appendix 1 for “EASA Form 1” substitute “CAA Form 1”.\n(3) In Appendix 3—\n(a) on the first page of the form—\n(i) for “[Member State (*)] A Member of the European Union (**)” substitute “United Kingdom”;\n(ii) for “[Member State Code (*)]” substitute “United Kingdom”;\n(iii) for “[Competent authority of the Member State (*)]” substitute “Civil Aviation Authority”;\n(iv) for “For the competent authority: [Competent authority of the Member State (*)]” substitute “For the CAA”;\n(v) for “EASA Form 3-145 Issue 4” substitute “CAA Form 3-145 Issue 1”;\n(vi) omit the notes at “(*)” and “(**)”;\n(b) on the second page of the form—\n(i) for “[Member State Code (*)]” substitute “United Kingdom”;\n(ii) for “For the competent authority: [Competent authority of the Member State (*)]” substitute “For the CAA”;\n(iii) for “EASA Form 3-145 Issue 4” substitute “CAA Form 3-145 Issue 1”;\n(iv) omit the note at “(*)”.",
