(a) the place where the operation took place;
(b) the date and time of the operation;
(c) the class and description of the beer used in the operation;
(d) the quantity and strength of the beer used in the operation;
(e) the description of the substance added to the beer;
(f) the quantity of the substance added to the beer;
(g) the quantity and strength of the resultant product.
