  "text": "In relation to the cancellation or suspension of a registered person’s registration under section 17, 18, 30 or 31 of the Act, the prescribed information, subject to paragraph 5, is—\n(a) a description of the regulated activity to which the cancellation or suspension relates;\n(b) the name of the service provider in respect of that regulated activity;\n(c) where the cancellation or suspension relates to a registered manager, the name of the registered manager;\n(d) an explanation of why the registration has been cancelled or suspended and, where applicable, the period of suspension; and\n(e) the relevant address.",
