Particulars for transmission to the proper consular officer

The name and address of the child.

The date, place of birth and nationality of the child.

The name and address of the applicant for the licence.

The name and address of the parent of the child.

Particulars of the engagement including where and for how long the child is to participate.

A copy of the contract of employment or other document showing the terms and conditions on which the child is engaged.

A copy of the licence.”.
