  "text": "(1) Appendix 10 (EASA Form 55) is amended as follows.\n(2) In the heading, for “EASA Form 55” substitute “CAA Form 55”.\n(3) On the first page of the form—\n(a) for “[Member State] (*)” substitute “CAA”;\n(b) omit “A Member of the European Union (**)”;\n(c) for “[Member State code (*)]” substitute “United Kingdom”;\n(d) for “[Competent Authority of the Member State]” substitute “Civil Aviation Authority”;\n(e) for “competent authority: [competent authority identification (*)]” substitute “CAA”;\n(f) for “EASA Form 55a issue 2” substitute “CAA Form 55a issue 1”;\n(g) omit the notes at “(*)” and “(**)”.\n(4) On the second page of the form—\n(a) for “[Member State] (*)” and “A Member State of the European Union (**)” substitute “the United Kingdom”;\n(b) for “[Member State code (*)]”, in both places it occurs, substitute “United Kingdom”;\n(c) for “[competent authority identification (*)]” substitute “the CAA”;\n(d) for “EASA Form 55b Issue 2” substitute “CAA Form 55b Issue 1”;\n(e) omit the notes at “(*)” and “(**)”.",
