  "text": "(1) In Appendix 3, the form is amended as follows.\n(2) On the first page—\n(a) for “European Union (*)” substitute “United Kingdom”;\n(b) in each place the words occur, for “Competent Authority” and “[Competent Authority]” substitute “Civil Aviation Authority”;\n(c) omit the note at “(*)”;\n(d) for “EASA Form 143” substitute “CAA Form 143”.\n(3) On the second page—\n(a) for “[Competent Authority]” substitute “CAA”;\n(b) omit “For the Member State / EASA”;\n(c) for “EASA Form 143” substitute “CAA Form 143”.",
