# The Aviation Safety (Amendment etc.) (EU Exit) Regulations 2019, Regulation 332: Amendment of Appendix 4 to Annex 6 to Commission Regulation (EU) No 1178/2011


(1) Appendix 4 is amended as follows.
(2) In the introduction—
(a) for “EASA Form 145” substitute “CAA Form 145”;
(b) in the first place the words occur, for “competent authority” substitute “CAA”;
(c) for “competent authority of issue” substitute “CAA”.
(3) In the form—
(a) on the first page—
(i) for “European Union (*)” substitute “United Kingdom”;
(ii) in the first place the words occur, for “Competent Authority” substitute “Civil Aviation Authority”;
(iii) for “[competent authority]” substitute “Civil Aviation Authority”;
(iv) omit the note at “(*)”;
(v) for “EASA Form 145” substitute “CAA Form 145”;
(b) on the second page—
(i) for “[competent authority]” substitute “CAA”;
(ii) for “For the Member State/EASA” substitute “For the CAA”;
(iii) for “EASA Form 145” substitute “CAA Form 145”.
