Paragraph 36 Education and Employment particulars
In respect of any person who was working or temporarily away from work in the last 7 days—
(a)
the number of hours a week usually worked in the main job;
(b)
whether mainly working at a workplace or reporting to a depot, at or from home, at an offshore installation or at no fixed place;
(c)
the address for the workplace or depot;
(d)
the mode of transport used for the longest part, by distance, of the usual journey to work.
