(a) White;
(b) Chinese;
(c) Irish Traveller;
(d) Indian;
(e) Pakistani;
(f) Bangladeshi;
(g) Black Caribbean;
(h) Black African;
(i) Black Other;
(j) Of mixed ethnic group, if so, stating which groups;
(k) Other ethnic group (stating which).
