Paragraph 10
Where a Schedule 4 or Schedule 5 body has undertaken an assessment of the person under the system known as the Common Assessment Frameworkf1—
(a)
the name and contact details of the person who has undertaken the assessment;
(b)
the date on which the assessment was undertaken, and
(c)
the date on which actions arising from the assessment are considered completed.
