  "text": "(1) Subject to sub-paragraph (2), the name and contact details of any person providing primary medical services under Part 1 of the National Health Service Act 2006 (promotion and provision of the health service in England) to the person.\n(2) Where the primary medical services comprise only a sensitive service, sub-paragraph (1) shall not apply and paragraph 9 applies.",
