Regulation 9 Form of permit
(1)
A permit must be in the form set out in the Schedule.
(2)
If a person to whom a permit is issued has a change of name or wishes to be known by another name, that person may send the permit to the Licensing Board with–
(a)
a fee of £25; and
(b)
a request that a new name be substituted for the old name.
(3)
A Licensing Board receiving a request in terms of paragraph (2) is to comply with that request and return the permit to the holder.
