An application under this Part must be—

(a)

made by the applicant to the administrator;

(b)

made on or before 30th April of the year following the first scheme year to which the application relates;

(c)

signed by or on behalf of the applicant to confirm that the information in the application is true and accurate; and

(d)

submitted in hard copy and by electronic means.
