  "text": "(1) There shall be inserted in Schedule 5 to the Rules, following Form 4.30 (Scot), the form set out in Part 1 of the Schedule to this Order.\n(2) For Page 2 of Form 4.4 (Scot) in Schedule 5 to the Rules substitute the page set out in Part 2 of the Schedule to this Order.",
