# The Enterprise Act 2002 (Consequential Amendments) (Prescribed Part) (Scotland) Order 2003, Article 10: Amendments to Schedule 5 – Forms


(1) There shall be inserted in Schedule 5 to the Rules, following Form 4.30 (Scot), the form set out in Part 1 of the Schedule to this Order.
(2) For Page 2 of Form 4.4 (Scot) in Schedule 5 to the Rules substitute the page set out in Part 2 of the Schedule to this Order.
