  "heading": "Reports of well operations to be submitted pursuant to Article 15(4)",
  "text": "The reports to be submitted to the competent authority pursuant to Article 15(4) shall contain at least the following information:\n(1) the name and address of the operator of the well;\n(2) the name of the installation and the name and address of the operator or owner;\n(3) details that identify the well and any association with installations or connected infrastructure;\n(4) a summary of the operations undertaken since the commencement of operations or since the previous report;\n(5) the diameter and true vertical and measured depths of:\n(a) any hole drilled; and\n(b) any casing installed;\n(6) the drilling fluid density at the time of making the report; and\n(7) in the case of operations relating to an existing well, its current operational state.",
