  "text": "ANNEX II\nINFORMATION TO BE NOTIFIED BEFORE ENTRY INTO THE PORT OF …\n(Port of destination as referred to in Article 6 of Directive 2000/59/EC)\n1.\nName, call sign and, where appropriate, IMO identification number of the ship:\n2.\nFlag State:\n3.\nEstimated time of arrival (ETA):\n4.\nEstimated time of departure (ETD):\n5.\nPrevious port of call:\n6.\nNext port of call:\n7.\nLast port and date when ship-generated waste was delivered, including the quantities (in m3) and the types of waste that were delivered:\n8.\nAre you delivering (tick appropriate box):\nall ☐\nsome ☐\nnone ☐\nof your waste into port reception facilities?\n9.\nType and amount of waste and residues to be delivered and/or remaining on board, and percentage of maximum storage capacity:\nIf delivering all waste, complete second and last columns as appropriate. If delivering some or no waste, complete all columns.\nSewage may be discharged at sea in accordance with Regulation 11 of Marpol Annex IV.
