That entry must comprise at least the following information:

1. (a)

   the number of animals being moved;

2. (b)

   the identification code of the holding of departure;

3. (c)

   the date of departure;

4. (d)

   the identification code of the holding of arrival;

5. (e)

   the date of arrival.
