  "text": "(a) Section ‘B. CZECH REPUBLIC’ is amended as follows:\npoint 1 is replaced by the following:\n1.. ‘Benefits in kind:\n(a) in general:\nthe health insurance company (according to choice);\n(b) for the air rescue service:\nMinisterstvo zdravotnictví (Ministry of Health).’;\n(b) Section ‘J. ITALY’ is amended as follows:\n(i) point 3.A is replaced by the following:\nA.. ‘Employed persons:\n(a) in general:\nIstituto nazionale della previdenza sociale (National Social Welfare Institution), provincial offices;\n(b) for workers in the entertainment business:\nEnte nazionale di previdenza e assistenza per i lavoratori dello spettacolo (National Welfare and Assistance Office for Workers in the Entertainment Business), Roma;\n(c) for journalists:\nIstituto nazionale di previdenza dei giornalisti italiani “Giovanni Amendola” (Giovanni Amendola National Welfare Institution for Italian Journalists), Roma.’;\n(ii) point 4 is replaced by the following:\n4.. ‘Death grants:\nIstituto nazionale della previdenza sociale (National Social Welfare Institution), provincial offices;\nIstituto nazionale per l'assicurazione contro gli infortuni sul lavoro (National Institution for Insurance against Accidents at Work), provincial offices; IPSEMA.’;\n(c) Section ‘K. Cash benefits:\nΥπηρεσίες Κοινωνικών Ασφαλίσεων, Υπουργείο Εργασίας και Κοινωνικών Ασφαλίσεων, Λευκωσία (Department of Social Insurance Services, Ministry of Labour and Social Insurance, Nicosia).\n3.. HUNGARY’ is amended as follows:\n(i) point I.1 is replaced by the following:\n1.. ‘Sickness and maternity:\nBenefits in kind and cash benefits:\nOrszágos Egészségbiztosítási Pénztár és a megyei egészségbiztosítási pénztárak (National Health Insurance Fund and the Regional Health Insurance Funds).’;\n(ii) point I.6 is replaced by the following:\n6.. ‘Family benefits:\nCash benefits:\n1.. Magyar Államkincstár Budapesti és Pest megyei Regionális Igazgatósága (Regional Directorate in Budapest and Pest County of the Hungarian State Treasury);\n2.. (Mutual Sickness Insurance Association), Amersfoort.’;\n(g) Section ‘R. AUSTRIA’ is amended as follows:\n(i) point 1(b)(ii) is replaced by the following:\n(ii) ‘in the case of treatment in a hospital for which a regional health fund is responsible, the regional health fund (Landesgesundheitsfonds) competent for the place of residence or place of stay of the person concerned;’\n(ii) point 2(b) is replaced by the following:\n(b) ‘In all other cases with the exception of the application of Article 53 of the implementing Regulation:\nPensionsversicherungsanstalt, Wien (Pension Insurance Institution, Vienna).’;\n(iii) point 3(a)(ii) is replaced by the following:\n(ii) ‘in the case of treatment in a hospital for which a regional health fund is responsible, the regional health fund (Landesgesundheitsfonds) competent for the place of residence or place of stay of the person concerned;’\n(iv) point 5 is replaced by the following:\n5.. ‘Family benefits:\nFinanzamt (Tax Office) competent for the place of residence or place of stay of the person concerned.’;\n(h) Section ‘S. POLAND’ is amended as follows:\n(i) point 2(a) is replaced by the following:\n(a) ‘for persons who have been recently employed or self-employed excluding self-employed farmers and for professional soldiers and officers who have completed periods of service other than mentioned in subpoints (c), (d) and (e):\n1.. Zakład Ubezpieczeń Społecznych (Social Insurance Institution — ZUS) — Branch Office in Łódź — for persons who have completed Polish and foreign periods of insurance, including periods completed lately in Spain, Portugal, Italy, Greece, Cyprus or Malta;\n2.. Zakład Ubezpieczeń Społecznych (Social Insurance Institution — ZUS) — Branch Office in Nowy Sącz — for persons who have completed:\n(a) exclusively Polish periods of insurance, residing in the territory of Austria, the Czech Republic, Hungary, Slovakia or Slovenia;\n(b) Polish and foreign periods of insurance, including periods completed lately in Austria, the Czech Republic, Hungary, Slovakia or Slovenia;\n3..
