Annex II EXPORT NOTIFICATION

The following information is required pursuant to Article 8:

1. 1..

   Identity of the substance to be exported:

   1. (a)

      name in nomenclature of the International Union of Pure and Applied Chemistry;

   2. (b)

      other names (e.g. ISO name, usual names, trade names, and abbreviations);

   3. (c)

      European Inventory of Existing Chemical Substances (Einecs) number and Chemical Abstracts Services (CAS) number;

   4. (d)

      F1a classification code in accordance with section 8 of the Taxation (Cross-border Trade) Act 2018;

   5. (e)

      main impurities of the substance, when particularly relevant.

2. 2..

   Identity of the mixture to be exported:

   1. (a)

      trade name and/or designation of the mixture;

   2. (b)

      for each substance listed in F2Part 1, 2 or 3 of the GB PIC list, percentage and details as specified under point 1;

   3. (c)

      F3a classification code in accordance with section 8 of the Taxation (Cross-border Trade) Act 2018.

   Identity of the article to be exported:

   1. (a)

      trade name and/or designation of the article;

   2. (b)

      for each substance listed in F4Part 1, 2 or 3 of the GB PIC list, percentage and details as specified under point 1.

4. 4..

   Information on the export:

   1. (a)

      country of destination;

   2. (b)

      country of origin;

   3. (c)

      expected date of first export this year;

   4. (d)

      estimated amount of the chemical to be exported to the country concerned this year;

   5. (e)

      intended use in the country of destination, if known, including information on the category(ies) under the Convention under which the use falls;

   6. (f)

      name, address and other relevant particulars of the natural or legal importing person;

   7. (g)

      name, address and other relevant particulars of the exporter.

   F5Designated National Authority:

   1. (a)

      the name, address, telephone and telex, fax number or e-mail of the F6Designated National Authority from which further information may be obtained;

   2. (b)

      the name, address, telephone and telex, fax number or e-mail of the designated authority in the importing country.

   Information on precautions to be taken, including category of danger and risk and safety advice.

   A summary on physicochemical, toxicological and ecotoxicological properties.

   Use of the chemical in F7Great Britain:

   1. (a)

      uses, category(ies) under the Convention and F8Great Britain subcategory(ies) subject to control measure (ban or severe restriction);

   2. (b)

      uses for which the chemical is not severely restricted or banned (use categories and subcategories as defined in F9Parts 1, 2 and 3 of the GB PIC list);

   3. (c)

      estimation, where available, of quantities of the chemical produced, imported, exported and used.
