  "text": "ANNEX 52-01\nEU form 302\n(1) The EU form 302 must conform to the specimen set out in this Annex.\n(2) The EU form 302 shall be made out in English or French.\n(3) If completed by hand, entries must be clearly readable.\n(4) Each EU form 302 shall bear a serial number, printed or otherwise, by which it can be identified.",
