Article 79 Obligations on the competent authority as regards the identification document of kept birds in travelling circuses and animal acts

The competent authority shall issue an identification document as provided for in Article 117(b) of Regulation (EU) 2016/429 for group of kept birds in travelling circuses or animal acts intended to be moved to another Member State.
The competent authority shall ensure that the identification document provided for in paragraph 1 contains the following information:
the name, address and contact information of the operator responsible for the birds;
the species of the birds;
the identification code, the means of identification and its location if applied to the birds;
details of the vaccination of the birds, if applicable;
details of the treatments of the birds, if applicable;
details of diagnostic tests;
the name and address of the competent authority issuing the identification document;
the date of issue of the identification document.
