# Commission Delegated Regulation (EU) 2020/2154 of 14 October 2020 supplementing Regulation (EU) 2016/429 of the European Parliament and of the Council as regards animal health, certification and notification requirements for movements within the Union of products of animal origin from terrestrial animals (Text with EEA relevance), Annex: Information to be contained in the animal health certificate accompanying movements within the Union of consignments of products of animal origin from terrestrial animals

Extent: E+W+S+N.I.

(a) the name and address of the consignor and the consignee;
(b) the name and address of the establishment or place of dispatch;
(c) the name and address of the establishment or place of destination;
(d) a description of the product of animal origin, including:
(i) the category of the product, as defined in Article 2 of Delegated Regulation (EU) 2020/687:
fresh meat (specifying the category, if needed);
meat products;
casings;
raw milk, colostrum and colostrum-based products;
dairy products;
eggs;
egg products;
composite products (indicating ingredients of animal origin)
(ii) the animal species from which the product of animal origin was obtained;
(iii) the Member State or region of origin of the raw material;
(iv) the treatment(s) that the product of animal origin has undergone;
(v) the marking applied on the product of animal origin, where required;
(vi) the place and date of their production or processing;
(e) the quantity of the product of animal origin;
(f) the date and place of issue of the animal health certificate; the name, capacity and signature of the official veterinarian; and the stamp of the competent authority of the place of origin of the consignment;
(g) the name of the listed disease or emerging disease causing the movement restrictions in the [establishment, food business or zone at the place of dispatch;
(h) details of compliance with the conditions for the granting of the authorisation referred to in Article 2 of this Regulation, and either:
(i) the title and date of publication in the Official Journal of the European Union of the relevant legal act adopted by the Commission providing those conditions; or
(ii) the reference to the legal act or instruction approved and made public by the competent authority providing those conditions.
