  "text": "A record in the form of a register showing in respect of each child placed with foster parents—\n(a) the date of his placement;\n(b) the name and address of the foster parent;\n(c) the date on which he ceased to be placed there;\n(d) his address prior to the placement;\n(e) his address on leaving the placement;\n(f) his responsible authority (if it is not the fostering service provider);\n(g) the statutory provision under which he is placed with foster parents.",
