Where it is applicable in relation to him—
(a)
his qualifications and where they were obtained;
(b)
if he is a healthcare professional, which profession, the licensing body which regulates it, his professional registration number and date of first registration in which the register maintained by that licensing body; and
(c)
if he is included in a primary care list, which list and the name and address of that PCT.
