# The General Teaching Council for England (Additional Functions) Order 2000, Paragraph 15


Where the teacher qualified as a teacher other than by successfully completing a course for the initial training of teachers—
(a) the type of training programme completed;
(b) the name of the school or institution at which his teacher training was undertaken; and
(c) the date on which the training programme was completed.
