# The Greater London Authority Elections (Amendment) Rules 2004, Paragraph 4


After Form 9 insert—
“Form 9A—Declaration of identity (GLA election combined with a relevant election or referendum)—(joint issue and receipt of postal ballots)
Form 9B—Declaration of identity (GLA election combined with a relevant election or referendum)—(separate issue and receipt of postal ballots)”
