  "text": "An application for a licence shall—\n(a) be made to the appropriate authority;\n(b) be in writing;\n(c) contain such information as is necessary for the completion of the boxes numbered 4, 14, 15, 16, 17, 18, 20 and 24 of the licence replicated in the Annex to Commission Regulation 1093/2001; and\n(d) contain such other information, and be accompanied by such documents, as the appropriate authority may reasonably require.",
