The notice of appointment for the purposes of an appointment under paragraph 22 shall be in [FORM 2.4]Form 2.9B or Form 2.10B, as appropriate.

(2)

The copies of the notice filed with the court shall be accompanied by—

(a)

the administrator’s written statement in Form 2.2B;

(b)

the written consent of all those persons to whom notice was given in accordance with paragraph 26(1) unless the period of notice set out in paragraph 26(1) has expired; and

(c)

a statement of the matters provided for in paragraph 100(2), where applicable.
