The form contained in Part II of this Schedule shall be added to Schedule 5 to the principal Rules as Form 4.29 (Scot).

(2)

The forms contained in Part III of this Schedule shall be substituted for the forms bearing the identical numbers in Schedule 5 to the principal Rules.
