  "text": "An applicant for an integration loan must provide the following information—\n(a) his full name;\n(b) other names by which he is or has been known;\n(c) his date of birth;\n(d) his address in the United Kingdom;\n(e) his telephone number (if any);\n(f) his email address (if any);\n(g) evidence that demonstrates the applicant satisfies regulation 4(1);\n(h) his national insurance number;\n(i) details of his income;\n(j) details of his assets;\n(k) details of his liabilities;\n(l) details of his outgoings;\n(m) confirmation as to whether any of his dependants or any member of his household has made an application for or received an integration loan;\n(n) the information in paragraphs (a) to (f) and (h) to (l) in relation to any dependants;\n(o) the amount he is seeking by way of an integration loan.",
