rule 5 words inserted by Enterprise and Regulatory Reform Act 2013 (Bankruptcy) and the Small Business, Enterprise and Employment Act 2015 (Consequential Amendments) 2016 Sch. 2 para. 1(3)

An application for registration or renewal of registration shall be made in Form K1, K2, K3, K4, K5, K7 or K8, whichever is appropriate.
