  "text": "(1) Persons claiming to be eligible to receive repayments under section 9 of the Act shall apply to the liquidator in such form as he may require.\n(2) Such persons shall furnish the liquidator with, and if so required shall verify, such information as appears to him to be reasonably necessary to establish their eligibility or the amount of the payment to be made to them.",
