In Form 6—

(a)

in the heading after “(DPMC Act 1978, s 6(9)(c)” insert “ OR PARAGRAPH 11(2)(c) OF SCHEDULE 6 TO THE CIVIL PARTNERSHIP ACT 2004”; and

(b)

in the Statement of Means of Child after “□ Married” insert “□ Civil Partner”.
