  "text": "After Form 28 in Appendix 1 there shall be inserted the following—\n“FORM 29\nDECLARATION AS TO MARITAL STATUS UNDER SECTION 55 OF THE FAMILY LAW ACT 1986\nRule 109\nFORM 30\nDECLARATION AS TO PARENTAGE UNDER SECTION 56(1)(a) OF THE FAMILY LAW ACT 1986\nRule 109A\nFORM 31\nDECLARATION AS TO LEGITIMACY OR LEGITIMATION UNDER SECTION 56(1)(b) and (2) OF THE FAMILY LAW ACT 1986\nRule 110\nFORM 32\nDECLARATION AS TO AN ADOPTION EFFECTED OVERSEAS UNDER SECTION 57 OF THE FAMILY LAW ACT 1986\nRule 110A”.",
