  "text": "(Note 1)\nName of the receiving qualifying nuclear facility:\nMBA code of the receiving qualifying nuclear facility:\nReporting period from\nto\nDate\nName, address and, if known, MBA code of the qualifying nuclear facility which is shipping the waste\nConditioned form 2.\nQuantity 3.\nRemarks\ng of P\ng of U-235\ng of U\ng of T\ng of P\ng of U-235\ng of U\ng of T\ng of P\ng of U-235\ng of U\ng of T\ng of P\ng of U-235\ng of U\ng of T\nDate and place of dispatch of report:\nName and position of signatory:\nSignature:\nExplanatory notes\n1.\nThis report is required for conditioned waste which has been received from installations/ qualifying nuclear facilities or from installations outside the United Kingdom.\n2.\nThe ‘Conditioned form’ column must show the conditioned form of the waste, e.g. glass, ceramic, cement or bitumen.\n3.\nThe quantity column may be based on the quantity data recorded at the qualifying nuclear facility and does not require measurements of the items imported/received.\nThis form, duly completed and signed, must be sent to the ONR in accordance with regulation 35.",
