  "text": "Name of the qualifying nuclear facility which is shipping:\nMBA code of the qualifying nuclear facility which is shipping:\nReporting period from\nto\nDate\nMBA code of the receiving qualifying nuclear facility or name and address of the receiving facility outside the United Kingdom 2.\nConditioned form 3.\nQuantity 4.\nRemarks\ng of P\ng of U-235\ng of U\ng of T\ng of P\ng of U-235\ng of U\ng of T\ng of P\ng of U-235\ng of U\ng of T\ng of P\ng of U-235\ng of U\ng of T\nDate and place of dispatch of report:\nName and position of signatory:\nSignature:\nExplanatory notes\n1.\nThis report must include all the shipments or exports of conditioned waste to facilities or qualifying nuclear facilities outside the United Kingdom that have occurred during the reporting period.\n2.\nFull name and address to be filled in for exports.\n3.\nThe ‘Conditioned form’ column must show the conditioned form of the waste, e.g. glass, ceramic, cement or bitumen.\n4.\nThe quantity column may be based on the quantity data recorded at the qualifying nuclear facility and does not require measurements of the items exported/shipped.\nThis form, duly completed and signed, must be sent to the ONR in accordance with regulation 35.",
