(a) in subsection (1)(b), for “certificate issued by the applicant stating” substitute “declaration by the applicant”;
(b) in subsection (2)(a), for “certificate” substitute “declaration”;
(c) in subsection (5), in paragraphs (a) and (b), for “issue a certificate containing a statement” substitute “make a declaration”.
