  "text": "(i) The name, address and telephone number of the applicant.\n(ii) The address and telephone number of the premises on which the flock is to be kept.\n(iii) The name of the person in charge of the premises on which the flock is to be kept (if not the applicant).\n(iv) The approximate number of birds in the flock.\n(v) In the case of birds which are to be housed continuously, the number of houses which will be maintained for such birds.\n(vi) In the case of birds which are to have free access to more than one house, the number of groups of houses which will be maintained for such birds.",
