# The Poultry Laying Flocks (Testing and Registration etc.) Order 1989, Schedule 5: INFORMATION TO BE CONTAINED IN THE NOTIFICATION OF THE RESULTS OF TESTS ON SAMPLES GIVEN TO THE APPROPRIATE MINISTER BY A REGISTERED PERSON


The notification of the results of tests on samples given to the appropriate Minister by a registered person under article 10(1) shall contain the following information –
(i) the name and address of the registered person giving the notification;
(ii) a description of the type of sample tested;
(iii) the date on which such a sample was taken, the address of the premises from which it was taken and the identity of the house or group of houses on those premises from which it was taken;
(iv) the result of the test; and
(v) the name and address of the laboratory which carried out the test.
