# The Premises Licence (Scotland) Regulations 2007, Regulation 4: Application for a premises licence


(1) Subject to paragraph (2), an application for a premises licence is to be in the form set out in Schedule 3.
(1) An application for a premises licence lodged no later than 16th January 2009 may also include the additional material set out in Schedule 4.
