  "text": "A notification under section 39(1) of the Act must contain—\n(a) the name of the service provider;\n(b) the name and type of regulated service to which the notification relates;\n(c) the place or places to which the notification relates;\n(d) the date on which the notification is sent; and\n(e) the paragraph of section 39(1) of the Act or of regulation 10 under which the notification is sent.",
