  "text": "(1) A request must be made in writing in English.\n(2) A request must include the requester's—\n(a) name;\n(b) address;\n(c) correspondence address, if different; and\n(d) proof of identity, if the requester is an individual.\n(3) Where a person is acting on behalf of the requester, that person must provide confirmation of authority to act on behalf of the requester.\n(4) A request must specify whether it is for—\n(a) a variation under section 23;\n(b) revocation under section 23;\n(c) review under section 25;\n(d) revocation under section 27;\n(e) review under section 29.\n(5) A request must explain—\n(a) where the request is for a variation under section 23, why the designation should be varied and what variation is requested;\n(b) where the request is for revocation under section 23, why the designation should be revoked;\n(c) where the request is for review under section 25, why the requester's name should be removed from the relevant UN list;\n(d) where the request is for revocation under section 27, how the requester is affected by the ship specification and why the specification should be revoked;\n(e) where the request is for review under section 29, how the requester is affected by the designation and why the designation should cease.\n(6) A request must include relevant evidence of the explanation provided under paragraph (5), where such evidence is available to the requester.\n(7) Where a request includes any evidence which is not in English, the request must include a translation of that evidence into English, and the translation must—\n(a) include a signed declaration by the translator that the translation is an accurate translation of the evidence;\n(b) include the date of the translation and the full name and contact details of the translator.\n(8) Where the request is a further request under any of the sections mentioned in regulation 3, the further request must explain what the significant matter giving rise to the further request is, and why it is significant.",
