  "text": "(1) Where an application for planning permission is made, the applicant shall certify, in the appropriate form prescribed in Part 2 of Schedule 2 to this Order or in a form substantially to the like effect, that the requirements of article 6 have been satisfied.\n(2) If an applicant has cause to rely on paragraph (4) of article 6, the certificate must state the relevant circumstances.",
