Regulation 49
In the Schedulef1—
(a)
for the forms numbered 1, 4 and 5 there shall be substituted respectively the forms numbered 1, 4 and 5 in the Schedule to these Regulations; and
(b)
there shall be inserted after the form numbered 5 the forms numbered 6, 7 and 8 in the Schedule to these Regulations.
