The report of the investigation which a local authority must submit to the court in accordance with section 44(5) of the Act must include—
(a)
confirmation that the Certificate of eligibility and approval has been sent to the CA of the State of origin in accordance with regulation 18;
(b)
the date on which the agreement under Article 17(c) of the Convention was made; and
(c)
details of the reports of the visits and reviews made in accordance with regulation 5 as modified by regulation 25.
