Article 4 Application form for train driving licences

1.

The F1... model of the application form, as set out in Annex IV to this Regulation may be used for train driving licence F2applications referred to in regulation 10 of the Regulations or regulation 10 of the NI Regulations. This model shall allow the inclusion of F3... requirements that may complement the information set out in Annex IV to this Regulation.

The ... model of the application form, may be used to apply for a new licence, the updating of licence particulars, a renewal or a duplicate.

Guidelines for filling in the ... model of the application form are set out in Annex IV.

F44.

The ORR and the Department may transfer the lay-out of the model of the application form into the procedures published by each of them under regulation 10 of the Regulations or regulation 10 of the NI Regulations.
