    the description of the material and the animal species of origin;

  2. (b)

    the category of the material;

  3. (c)

    the quantity of the material;

  4. (d)

    the place of dispatch of the material;

  5. (e)

    the name and the address of the consignor;

  6. (f)

    the name and the address of the consignee; and

  7. (g)

    details allowing the identification of the authorisation of the competent authority of destination.
