A notice relating to a registered provider must also identify the registered provider and state—

(a)

its principal trading address;

(b)

any name under which it was registered in the 12 months before the date of the housing administration proceedings; and

(c)

any name or style under which—

(i)

the registered provider carried on business; and

(ii)

any debt owed to a creditor was incurred.
