all relevant provisions to which the lift conforms;

where appropriate, reference(s) to harmonised standard(s) used;

where appropriate, the name, address and identification number of the F4approved body which carried out the F5Type examination of lifts set out in Part B of F6Schedule 11 and the reference of the Type examination certificate issued by that approved body;

where appropriate, the name, address and identification number of the approved body which carried out the unit verification for lifts set out in F7Schedule 15;

where appropriate, the name, address and identification number of the approved body which carried out the final inspection for lifts set out in F8Schedule 12;
